保護傘公司(Umbrella Corporation)的致命病毒繼續肆虐全球，把剩餘人類變成食人喪屍! 在企業秘密基地內，甦醒後的愛麗絲(米娜祖華域芝 飾)逐步尋回記憶，但真實與記憶竟混亂不清。世界末日正在倒數，全人類很快滅絕，愛麗絲和新盟友中國女特工Ada Wong(李冰冰 飾)、Rain Ocampo(米雪洛迪格斯 飾)等，先後到東京、紐約、華盛頓和莫斯科，誓要揪出散播病毒的幕後黑手，揭發背後隱藏著全人類的絕望真相!
